AAC and Alternative Communication: Every Person Can Communicate
There is a belief that runs deep in many families, and in many clinical settings, that speech is the goal. That if a child is not talking, the job is to make them talk, and that everything else is a consolation prize for children who could not get there.
That belief causes real harm. It delays access to communication tools that children need right now. And it rests on a fundamental misunderstanding of what communication actually is. Communication is not the same as speech. It never was. Every single person communicates: through gesture, expression, pointing, writing, and technology.
Augmentative and Alternative Communication, or AAC, is the umbrella term for the full range of tools and strategies that help people express themselves when speech alone is not sufficient. It is one of the most important, and most misunderstood, areas in all of speech-language pathology.
The main categories of AAC
AAC systems are broken down into several functional tracking pillars, ranging from low-tech baseline solutions to advanced dynamic eye-tracking software:
PECS (Picture Exchange Communication System)
A child hands a picture to a communication partner to make a request. It is immediately functional: a child can communicate a want before they have any words, and it builds the core purpose of communication, which is exchanging meaning with another person.
Low-tech boards
Paper-based symbol systems: a board with pictures, symbols, or words organized so a person can point to express themselves. No battery required, always available. A well-designed low-tech board is frequently underestimated. It should not be.
Speech-generating devices
High-tech devices and apps where a user selects symbols or text and the device speaks the message aloud. These tools can support fully robust language systems, not just basic requests.
Spelling and typing
Letter boards, keyboards, and assistive typing technology. Many nonspeaking or minimally speaking individuals, particularly autistic people, are highly literate. For some, access to a letter board or keyboard has been transformational, unlocking communication that others never knew was there.
Sign language
ASL, Makaton, or personalized sign systems used alongside or instead of speech. The research is clear: signing does not inhibit speech development. It frequently supports it.
Eye gaze technology
For individuals with significant physical differences, eye-tracking software allows them to select messages on a screen using only their eye movements. The technology has advanced enormously and opens communication to people who have very few other motor options.
The myths that keep children from getting what they need
The myths around AAC are persistent, and they cause real delays in getting children the physical communication support they require.
Myth: The device will stop them from trying to talk
This is probably the most persistent myth in the field. The research on it is consistent: AAC does not inhibit speech development. A comprehensive review published in the American Journal of Speech-Language Pathology found that AAC use actually supports and in many cases accelerates speech development. Giving a child a reliable way to communicate reduces frustration, builds communication confidence, and models language, all of which support speech rather than undermine it.
Myth: My child is not ready for AAC yet
There is no prerequisite for AAC. None. A child does not need to demonstrate a certain level of cognitive functioning, eye contact, play skills, or symbolic understanding before they can benefit from communication support. The American Speech-Language-Hearing Association explicitly states that readiness is not a valid criterion for withholding AAC. Every child deserves access to the tools that allow them to communicate.
Myth: AAC is only for kids who cannot talk at all
Many AAC users have some speech. They use AAC to supplement their communication in situations where speech alone is not enough: when they are tired, stressed, sick, or in a noisy environment. A child who can say some words but cannot reliably communicate complex thoughts or needs is a strong candidate for AAC support. Communication tools exist on a spectrum, and most individuals who use AAC use multiple modalities depending on the context.
Myth: Typing and spelling are too cognitively demanding
Many nonspeaking autistic individuals are highly literate. The barrier is not cognitive; it is motor. Getting intentional, reliable motor output is the challenge, not understanding language or text. When the right support is in place, spelling-based communication methods can unlock expressions that nothing else has reached.
What SLPs actually do with AAC
An SLP's role with AAC is to find the right system, or combination of systems, for a specific individual based on their motor abilities, cognitive profile, sensory needs, communication environment, and goals. They then teach the child, their family, and their school team how to use it.
AAC is not a matter of give a device and walk away. It requires consistent modeling, a robust vocabulary, and a communication-rich environment at home and at school.
SLPs use a strategy called Aided Language Stimulation, in which the adults in a child's life use the same AAC device or board alongside the child, demonstrating how it works, modeling language, and building a shared communication culture. Research supports this approach as one of the key factors that determines whether an AAC system is actually used and generalized over time.
Communication is not something a child has to earn. It is not a milestone you unlock after proving you are ready. It is a fundamental human right.
Communication is a right, not a reward
The most important shift in thinking about AAC is to presume that there is something worth saying, and then to provide the tools that make saying it possible.
Speech is not the sole goal. Communication, in whatever form works for this child right now, always is. Every voice matters.
